Jim,
you dont say if the phimosis is a problem or merely the lack of hygiene? how old are you?

If you have phimosis, can you see a doctor and ask for betamethasone or a minimal dorsal slit

if you have embarrassment about even seeing a doctor with a smelling foreskin then I can imagine swilling the area out thoroughly will take time and thought, ... and a bath ...and a collection of q-tips,
