Ripped frenulum what should i do next?

I'm 29 years old and have had an active sex life for 13 years which was very pleasurable until 10 months ago when sex suddenly became pain full. I visited my Dr. recently and he advised two options, either (and slightly drastically a circumcision or the use of xylocaine cream. Obviously I don't fancy either option, as I believe the cream will just mask the symptoms and I feel cicumsion is an extreme option. I don't think I have any other problems ie frenulum breve as its only a recent problem can anybody out there please help me ? thanks Steve.
