Steve, you may consider circumcision an "extreme option," but it certainly will put a stop to your pain problem once the circumcision has healed. Dorsal slit might provide the same solution but I think something still needs to be done about the frenulum (just as it would have to be with a full circumcision). Also, I wonder why you refuse out of hand to consider the pain-killing cream. Unless you make an effort of one kind or another, you'll get no change for the better. Anonther appproach that may allow the raw tissues of the frenulum to heal is to keep the foreskin fully retracted ("skinned back") for 2 or 3 weeks, to allow air to get to it until the torn tissue has healed. You obviously have something there that either is not healing or is re-torn every time you have sex. There should be s simple, final, solution for it. By the way, there are much worse things than circumcision! RJK.
