Sounds like his glans and foreskin haven't started to separate yet. They just need a bit of help. There are two excellent methods of helping this along. Right now, every time he has a pee, have him pinch his foreskin shut so that his foreskin balloons up with the pee before he lets it come out. It may be tidier for him to sit when he does this as it will come out quite quickly when he lets it loose. If possible, he could do this a couple of times each pee. Slowly, the foreskin and glans adhesions will begin to separate.
The second method will come in several years when he enters puberty and begins to wank himself.

You need to find another doctor. Besides, the steroid cream is to loosen up the opening at the tip, it won't have any effect on the adhesions.

Stretching his foreskin whilst having a pee as I mentioned above will begin to have results in helping the adhesions separating. Any other treatment will mean a circumcision and in order to do that, they'll take a probe and run it up under his foreskin and literally skin him alive to separate the glans and foreskin. Doesn't seem like much of an alternative.
