What do you use for hygiene of the glans ? If you use soaps, stop. Just rinse the glans and a good massage with the hand in teh shower will do the right hygiene. Soaps leave residues etc that can cause that itch.
Also, have you tried leaving your foreskin retracted in underwear ?
Finally, as you get hard, you should retrcat ski right away and ensure glans has dried from its sticky wetness before engaging in action or putting on condom.
