I would like to comment on the embarrassment problem. It is hard for the patient not to be embarrassed when the doctor him/herself is embarrassed. I find this amazing after my experience of doctors in the US. An American doctor views it as a point of pride never to flinch in front of a patient. They are trained to have the mental attitude of the toughest marine sargent and will betray neither embarrassment nor surprise no matter what you tell them.
On the other hand, you can see a British doctor cringe when you start to ask a question about the "nether regions." I think they need to shape up their training.
