Thank you very much for your valuable advice. I have tried to get advice from the medical profession but they don't seem to have the patience to sit and talk a while. Your contributions are therefore particularly welcome.
Please don't get me wrong, I would be happy to keep my frenulum except for the problems it is causing me. Let me put it bluntly: during sex without a condom, penetration causes retraction of the foreskin behind the corona where it gets stuck and the frenulum pulls so hard on the meatus that it does a good job of closing off the opening so that ejaculate can not easily get out and the back pressure causes pain and discomfort. Even if I did not have current problems following the tear I still think it would have to go.

As a bit of an aside: there are many photographs of normal and abnormal penises on this web site but none give a good view of a normal frenulum as they are all taken from the opposite side. I would appreciate a clear photograph of a normal frenulum from its side of the shaft. How do you define the two sides ... are they dorsal and ventral perhaps? If the side is defined with the penis hanging down the frenulum would be on the dorsal side. If defined with the penis standing up it would be on the ventral side. Please help me get the terminology straight.

I am a bit disturbed that you have interpreted my description of the tiny ulcers as herpes. Please bear with me on this one because all my questions have almost been answered and I won't keep going on like this. In the light of the following facts do you still think there is a possibility of herpes:
1 I have never had herpes in the past to my knowledge. I understood herpes to be painful and that one is unlikely to be unaware of it. Also, there is no pain specifically associated with the "ulcers."
2 The little "ulcers" or "raw red spots" develop within an hour of sexual activity and go away within 24 hours or so with no pain or scabbing.

As you say Paul, my description does not fit with a simple tear of the frenulum, which is why I am having so much difficulty getting to the bottom of this and getting it sorted out. The tear itself has long since healed and does not seem to represent a "weak" point. Neither does the frenulum seem to be any shorter than previously. No, the problem is that any degree of retraction pulls on the meatus which promptly gets painful and inflamed. (I would be happy to start a program of slow stretching except that it won't tolerated it any more). I would like to think that you are right and that there is a chronic infection because that could be treated with an antibiotic. However, I have already had 2 ten day courses of doxycycline and the problem persists.
Any final advice before I go to a private doctor would be most welcome.
