Ring and ask to speak to the doctor. Tell him who you are - you suggest he already knows you. Point out you do not want your parents told. He will figure it out for you. Realise it's his business to know that guys (and of course girls) masturbate, and sometimes do funny things and hurt themselves.

If he were to tell your parents against your wishes, then on the one hand, you may well have a case for suing him (even when you are a few years older) and (as he should realise) after you told all your school mates that he had breached confidentiality, he could reasonably expect to get no business from any of them in the future. I think that would be a good reason why he wouldn't tell.
