Three possibilities come to mind - either you have a degree of chronic inflammation in this spot - perhaps using soap where you shouldn't, or you have a small "web" of skin which is abnormally delicate (I would think the normal fraenulum would be tougher than this), or you are simply bashing it far too hard - as your later comments hint.

If the part that tears is so thin you can see colour through it, you may indeed need to have a doctor remove it. A knowledgeable doctor (as against one who recommends circumcision) would be most helpful.

I suspect that you are masturbating without touching the penis head. Do you pull the skin forward and back over the head (usual way), or have you perchance fixed on masturbating with the skin pulled back over the shaft, using it to tug on the head?
You likely need to try a different technique. Use a lubricant, preferably a "personal" lubricant from the chemist intended for this purpose, but a vegetable oil would likely do, Vaseline as a last resort (very difficult to remove afterward, you see). Never, ever use soap or shampoo or automotive lubricant!
Using a lubricant, you surround your glans (penis head) with your hand and move it slowly around in whatever way you find pleasant. This (imitates intercourse,) will achieve your desired aim, very reliably, rather quickly, and without trauma.
