To be the devil's advocate. If you are able to retract your foreksin when soft, then tighter underwear will help sionce you would be able to keep your foreskin retracted in your underwear for a certain amount of time, and the more you do this, the faster the foreskin would loosen up and any "health" problems under the foreskin will clear up.

Note that if your foreskin is quite tight, you shoudln't leave it retracted as it may cause paraphimosis (strangulating the glans).
