Well, since you mention me, then I have to agree, Will. It sounds as if he has a tight foreskin and a tight fraenulum. Stretching would be appropriate for both, but undoubtedly more effective or at least, more rapidly effective on the foreskin (prepuce) than the fraenulum. However, ipso facto, stretching the foreskin generously will enable it to retract even with a fraenulum breve and that should therefore be tried first.
A fraenuloplasty might be resorted to if it appears just too tedious to stretch the fraenulum. I do not think there really is much special sensation contained in the fraenulum, but rather that it serves to convey movement from the foreskin to the particularly sensitive meatal area of the glans (just as the corresponding crura do in women as I habitually point out). A suitably performed fraenuloplasty can therefore preserve the desired level of function while complete removal may or may not. Circumcision on the other hand is quite inappropriate.
The more I consider it, the more I like the proposal of wearing dilators (with a central hole so it is possible to pee without removing it) for most or all of the day, as discussed in previous posts here.
