I can only suggest telling your parents you have a "personal" problem and want to see the doctor in private. Point out to the doctor that you do not wish the matter discussed with your parents. After that, the doctor is (supposed to be) entirely objective - he knows for example that most people masturbate, so you simply tell him straight out that you have a problem when you do so and (though he may smile) he will look into the problem. He also knows young people get erections easily, so if that happens (it makes the assessment easier in any case) he will not be concerned either, nor should you be.

Have you been working on the careful stretching at times other than masturbating?

Well, not as such. A dorsal slit is a rather rough-and-ready approach to not being able to pull the foreskin back over the glans, albeit nowhere near as brutal as circumcision, which is why you have always heard me recommend that circumcision is totally unnecessary in almost all circumstances as at worst a dorsal slit or (same thing done more neatly) preputioplasty could be done. But either procedure is unnecessary and inappropriate as stretching will achieve the same goal over a period of time and the effect will be permanent if the need to "over-stretch" in terms of time and degree is understood.

It sounds as if you are being put off the stretching by something, possibly a fraenulum breve, that causes bleeding when you try to stretch and frankly, this is such a peculiar situation that no-one can be more specific without actually seeing it, or at least a photograph. For this reason, you probably do need to see a doctor for assessment and some specific procedure such as stripping of adhesions so that you can stretch, possibly stretching the foreskin with some anaesthetic (and continuing to stretch afterward) or just perhaps, snipping of the fraenulum.

But particularly in America. there is a danger that a doctor will not be interested in attention to detail, but recommend a circumcision instead of fixing the real problem, in which case he would be indicating general ignorance of the subject.
