True. The clue would appear to be continuous pressure. As this applies to the foreskin, the concept of an "appliance", essentially worn continuously in the preputial opening would seem to be the ultimate. Obviously you didn't try that approach - it seems to be a very recent addition to the discussion, and I suspect will prove to be one of those serendipitous ideas (for which I claim no credit for originality) that suddenly blossom.

At present, this means devising one's own dilators, but given the number of appliances now advertised for foreskin restoration, I would expect a similar market to develop for a commercial one, probably a series of "grommets" or "bobbins" cast in silicone rubber. The closest ad-hoc off-the-shelf candidate for now appears to be a 35mm or (smaller) "APS" film canister, but this may be too big for a start.
