I need some advice, Please Help Me !

I am 17 years old and I have a problem in pulling my foreskin back. I seem to have to much foreskin, I only have a small hole at the top of my skin so I can only see just the very top of my head. I have tried sex and it has been very painful at times. I have known about this problem since I was 15 but I have been to scared to tell my mother.

The problem is that when I have an erection, I can only see just a small part of the top of my head as the hole in my foreskin is not large enough to go around my entire head.
The hole is not even a cm wide, so its very small but after sex and masterbation the hole seems to get larger but not that much.

Please could you give me some advice, I would really apreciate it. Please try not to use the complicated terms, as I dont really know them. I would feel pretty embarrased going to a doctor but I think I could tell my mother. What should I do ?
