Just read an article (in french) about the use of Betneval on phimosed foreskins to loosen them up.
An interesting note: they say that the foreskin must be partially retracted when applying the ointment to ensure that both sides of the tight skin gets the cream.
I guess the statement should really have been to ensure that the phimotic ring must be exposed fully in order to apply the ointment to it. (which makes sense).
This has interesting implications. If you have a tight phimosis that doesn't allow any significant retraction, then you cannot apply the ointment to the full surface of the tight ring of skin. If the phimosis is loose enough, you can usually pull the skin in such a way that when you then retract it just a bit, the phimositc ring will become exposed on an area of the glans that is small enough that the phimotic ring isn't yet tight relative to glans diameter.
And of course, in cases where the phimosis allows retraction when soft but not when hard, one only has to wait to become soft to retract the skin enough to expose the phimotic ring and apply the oitment to it.
But in the case of a pinhole phimosis, applying the ointment to the whole phimotic ring becomes more of a challenge. I guess one could apply it only to whatever you can and hope that progressively more and more of the phimotic ring can be exposed and coated with the ointment. (that would make the stretching much more lenghty).
