how to apply steroid cream for phimosis on my 7-yr-old??

Hello~
My 7-yr-old son has been diagnosed with true phimosis and I believe it's accurate, since the opening has become barely a pinpoint and now he is having voiding problems--ballooning and spraying (all over the place). We thought for a long time that it was common "unretractable foreskin" and were going to wait and see if it would correct on it's own. He's had several bouts of balanitis, but recently, he's had red scaly itchy skin on the very tip, AND most noteably, red circular patches on both inner thighs RIGHT where the tip of his penis would touch. I didn't put 2 and 2 together for a while but now I think it must be fungal and ever since that cleared up, the opening of the foreskin is a pin-point and interferes with voiding.
I don't want to take the drastic way out unless and until our options run out, so we're going to try the hydrocortisone cream first. And what I need to know is how to do it??
And any other advice is welcome~
