Not sure what you mean by true phimosis vs tight foreskin, but in a seven year old, it is ordinarily premature to be making that judgment. Many boys have unretractable foreskins well into their teens, some until early twenties.
If you are suspecting a fungal infection, why are you using steroids rather than a fungicidal preparation? Did the doctor comment on the infection, or did prescribe anything to clear it up? If it is fungal, it needs to be cleared up before you start stretching exercises.
You also need to determine what is causing the infection. Are you using soap around the preputial opening. Has the boy been tested for diabetes? What kind of diet does he have? I'd suggest getting some advice from your local health food store on diets and preparations that are helpful in clearing up candida if that is what is confirmed.
