What Can I Do

I am unable to fully retract my foreskin when flacid or erect because of skin at the inner tip of my foreskin which is attached to my penis. When flacid and I retract my foreskin, it becomes stuck behind the head of my penis and if I were to get an erection at this time, the attached skin would surely rip. This is causing me real problems and I am unable to penetrate for fear of tearing my foreskin. Can anything be done and if so what ?
