So, it would be heavily advisable to find the reason. Easily breaking skin always suggest serious presence of fungus, and should be treated accordingly. If you want to try something yourself, Canesten is an anti-fungal creme, and works quite well most of the time. But then again, it is advised that you go to your GP and have the skin checked. If they suggest (without actually looking at it, taking a biopsy, or ANYTHING they would do on other skin areas) circumcision, ask them whether they would have your toes chopped off, were you to get some skin infection there.

Modern human will probably find both substances filthy, but that's all there is to it. As long as you can pee without problems, most of the smegma (which is shed skin, bodily oils and some other stuff) you may produce (well, I know I haven't seen much on my dick lately!) will get flushed out with your pee.

You can always direct water from the shower into your foreskin, unless the showerhead is fixed on the wall or something, but then if you are concerned, the tap will do too.
