it was clearly written, read more thoroughly:

when you go to the toilet, sit on it [so you don't make a mess], and with your fingers, try to keep the end of the skin shut. pee gently and notice if the foreskin 'balloons up'. [if it balloons up okay, there should be no adhesions of significance.] if it's full enough, release and let the pee out, of course :)
