Long foreskin- can i get "a little off the top"?

I am 19 and can fully reract my foreskin , so no probs there.

the thing is I have to pull back my foreskin when i pee otherwise it gets messy.

when i am fully erect the head of my penis is still fully covered by my foreskin which is annoying when I am making love because it gets very sensitive.

I havnt spoken to my doctor yet, (am too embarrased) an I dont want to totally loose my foreskin, so can i just get a bit taken off? just enough for my head to show when erect, and can i keep the bit that attaches my foreskin to the rest of my penis?
