This is what I had suggested Dan and it seems to have worked quite well.
First things first, be gentle. Don't pull too hard. Slowly, gradually pull more forwards and then try to pull it apart so as if to open the hole, and try to find the point where you feel the tension in the skin (as would you when you pulled your lips in a joking fashion as a kid!), but try to not make things hurt. Dan has been doing this kind of thing once a day, sometimes twice, but he'll probably describe it in more detail, as he has the first-hand experience.Oh... and creams. They are not necessary, and no, they are not available OTC. And if a doctor is trying to sell you hydrocortisone, don't accept it, that's not good for this.
