I was with ChII's advice until he said to leave it retracted for long periods. Once you achieve the looseness you want, you won't have any problem with re-tightening unless you get an infection or fail to do anything with your penis. Letting the skin cover the glans protects sensitivity. Cleanliness is only a problem if you don't clean, and that applies with or without glans coverage. Oh, and the medicine to use is betamethasome, not a cortisone: I'm actually not clear whether corticosteroid is a term which applies to either of those, but those are the products you are likely to encounter.
