Just /how/ tight is it exactly when flaccid? A bit of a detail there would help our imaginations a lot.

Otherwise, you could just try the oft-mentioned full-forward-and-open method (not sure, perhaps you have been doing that!) and flavour it up with inserting your two little fingers in the hole. I think it should be possible to. And it makes it a whole lot easier to pull at the tight skin.
