One of the problems is that some creams help and some hurt the process, and I have heard of doctors giving the wrong one. Some just don't know that betamethasone is the only proper one and some don't care or want to cause a boy to have to be circumcised. Yes there are doctors with such anemotional attachment that they would mislead their patients into it when totally unnecessary.

The other problem is that the cream works with stretching to make the skin grow. It was found to work 'by itself' in sexually mature men, but it has to be taken into account that their erections provide substantial stretching for a tight foreskin. A five-year-old gets erections, but they're such little things, it won't stretch much. At that age, the best loosening exercise is just the natural playing and tugging on his little willy that boys will do if not strongly scolded and shamed against it (but they can be encouraged to do it in private).
