>Should I discuss this with my regular primary care doctor or should I get a referral to a urologist and then discuss this?To me, and tell me if I'm wrong here, the fact that you were never able to is simply caused by that you were never taught to (or, as a good amount of people with phimosis: were actively discouraged touching their penis) -- and that's all there is to it. So it seems to me that the primary reason for your phimosis is nothing pathological, but simply the lack of "training".
And usually, the pathological problems (such as BXO/LSA or a fungal infection) are pretty easy to notice ("hmm, something is different") although proper identification is to be done by a professional.
On a third note, the urologist is very likely to suggest that circumcision is the one and only solution for your problem.
