I think you (and the doctor) are on the right track. Adhesions, but not phimosis, should already have freed themselves by now, and I tend to favor, as you both do, the gradual approach, *IF* it shows any sign of progress. Your boy may have made some progress already, but also made it sore at the same time, causing the next session to be painful. Exercise reasomable patience during this process, to give it a fair chance to work. Pulling forward instead of backward *MAY* tug the fused skins surfaces apart at a better angle, but there is no way to know unless results appear. Try both ways. Also try the various stretching techniques intended for phimosis, any of which *MAY* tug at the fused skins in an effective manner. But forget the creams intended for phimosis. They are applied to the outside of a tight foreskin, and would be ineffective on adhesions on the inside.
