The most we know is that the head of your penis is very sensitive. That you have a problem with your frenulum, we don’t know. You have a tight foreskin, and I don’t think it does any good to it that you q-tip around inside (if I understood correctly, you were not only told to do so, but actually did it, is that correct?). So anyway... Not sure what to suggest, but here’s a bit of thought.

Since the head is very sensitive, it’s probably not at all comfortable to tug the foreskin when it’s not slippery inside. Either produce some precum or use some kind of lubrication, and try to do the forward pulling stretching exercises for a little while (say, a couple weeks), and let’s see if that changes something.

As for cleaning, Ivan usually suggests that one buys a syringe and uses it (without the needle of course!) to press water inside while the foreskin is too tight.

Seeing how long the waiting lists are and such, it might be a good idea to spend that time actively.
