A foreskin that knots itself up? So much so that he has to untie it? What kind of knot? A fisherman knot? Boot lace? Seriously, if this is a legitimate problem then I would suggest your brother visit his doctor and get some professional help as a self-knot-tying foreskin sound pretty serious!
