I think pain killers will work. You could try ice on it (just now remembered, that might even make it less swollen). In any case, yes, do go to a doctor, but one thing, circumcision is Most Usually Not Required to treat paraphimosis. I really wish you had come here earlier. In any case, do try to ice it and try again (press out the blood from the head, push the skin forward — you might want to lubricate the head so the foreskin can slip over it more easily.)
