The reason you don't feel any pain during masturbation is because your technique is probably modified so you don't put any force on the frenulum itself. I used to do this too without even realizing it. Sex on the other hand is a very frictional back and forth motion, which WOULD put stress on the frenulum. For a solution to your problem, some say that stretching the frenulum slowly over a period of time works. The stretching excersize used for stretching/lengthening the frenulum is shown below:

I'm not sure why you're so busy for the next 5 years that you can't see a professional about any other alternatives. It almost sounds like you're scared of going, which is perfectly normal. My suggestion would be to try the various stretching excersizes and see if you can fix the problem naturally. As far as hygiene is concerned, you should be washing under the foreskin. In your condition, though, I imagine this process is difficult. There are ways to flush the area out with water (don't use soap) such as a syringe (without the needle! yipes!). Heck, maybe even a turkey baster. Q-tips may also help. I think the best way to go about this is to take baths whenever you can, do the stretching excersizes while in the bath (hot water loosens the skin to aid in stretching). It will be easier to wash under the foreskin this way as well. That's all the advice I can give on natural remedies. If this fails, and nobody else on the forum has any other advice, you may need to find time to see a good professional.
