Circlist has some pictures of a penis with frenulum breve and a difficult to retract foreskin, so that might help. Betamethaone is used for loosening up the skin but it causes thinning of the tissue and cracking. Its mode of action is to weaken the underling fibroblastic structure; that's what causes stretch marks in adolescence and in people on steroids. Also, it lowers the resistance to infection on the area to which it is applied. So be careful. While the foreskin can be stretched (if you go to Circlist you will a very extreme example of this... a prepuce as long as the penis, which may have been natural but unlikely).
In any case, stretching must be a slow and carefully controlled process because on every occasion when the foreskin tears and bleeds, microscarring must take place. Eventually, the foreskin will become less pliable and indeed may begin to constrict further. Vit E cream is a safer way to go. Take it easy and in a few months you may be able to retract further. The white spots could be scar but from this distant vantage point it is hard to know! Sometimes, despite your best efforts, a circumcision might be needed; don't despair if that ever is suggested.
