Pinhole phimosis can be such that the owner doesn't feel any discomfort. Or it can be uncomfortable.

The guy may have developped his own technique for masturbation that avoided discomfort. But when he starts to have sex, it may be a different story he didn't realise would happen. He may not admit to having discomfort or pain, but it will restrict the way he makes love because instinctively, he will avoid any manoeuver that would cause discomfort. So yo may not get much in terms of stimulation.

Ironically, a guy with a deffective penis is more likely to go for more foreplay to delay the activities that can cause discomfort (penetration).

If it is decided to try to stretch a pinhole phimosis of an adult, it could take months. Don't believe in the miracles of stretching. It works well for mild cases of phimosis. But there are limits to how much the creams can do in a given amount of time.

If a circumcision is chosen, he'll be in full action within one month (limited action beforehand). While his glans will be raw the first week, it will adapt during the first month. He can wear a bandage over glans to protect it during first week,.

If he is concerned about losing his foreskin, he can have a partial circumcision that exposes only the tip of the head when soft. Takes a bit more care during healing to prevent scar from forming a new phimosis, but once healed, it is problem free.

There are also preputial plasty. They make small cuts into the phimotic ring to loosen it, but the endge of the foreskin (when soft) may look a bit odd, depending on the expertise of doctor.

If you opt for a circumcision, make sure he discusses with his doctor the exact results hw wants (how much skin to remove) and whether to keep the inner foreskin or not, keep frenulum or not. It is cosmetic surgery and the doctor shoudl be doing it according to the body owner's preferenxces, not impose his own preferences on the patient.
