First part: a pin-hole meatus is not likely to improve and is a true, absolute medical indication for circumcision, because unlike a "tight" foreskin, the true pin-hole has firm grissly tissue right at the preputial ring and it is rare for this to be able to be stretched: however... if you can do this- pull the foreskin back until the opening is stretched as far open as it can be without hurting, insert your cleaned, trimmed and lightly lubricated little fingertip against the opening and advance the foreskin about 1cm onto the finger. With your other hand use your thumb and forefinger to make a ring to clamp the foreskin in place on your little finger and slowly rotate the little finger back and forth while pushing gently towards the glans. I works best right after a hot bath (or in it if you can stop the foreskin from slipping away due to the water- this is altogether not easy, but it is doable). The intent is to stretch the phimotic ring up gently and the warmth and moisture really make this easier than doing it dry and cold! You need about 30 secs of pressure and rotation and it should be done on and off for 5 min to start building up to 10 mins three or four times a day. If you get an erection during the process it helps. This may take months but in addition, bet some steroid cream and apply a little into the "trunk" of the foreskin. Not more than 1/4ml and do this three to four times a day as this will start the thinning of the phimotic skin BUT remember, if this is a true phimotic ring, the problem lies below the skin and this is what makes this tough to stretch. Obviously do the stretching exercises before applying the cream and then apply right after. Be patient but after 6 months it absolutely will NOT happen without trauma. Here is why that is a problem: themucosa over the ring may crack during this stretching process and that will guarantee absolutely unfixable phimosis (if it is not already) and the only cure is a circumcision. Do not be afraid of this if it is necessary but get back to us if you need help in that direction. Hope this helps.
