The method described by Dave is pretty spot-on, I think.

No, it’s a good idea. Optimally, urine in itself is a good cleaning substance (being practically steril in a healthy man) — its real problem is that once it’s outside, it’s a fantastic breeding ground for bacteria. So yes, water is a good idea.

I have some questions.

It is suspicious that the opening would get smaller — this doesn’t /just/ happen, there is usually a cause. A trip to a dermatologist might help in getting to know the exact problem (not just the obvious result).

Do you have diabetes?

Is the pattern of the tip of the foreskin any different from what it used to be? Has the colour changed? Is any of these different from what you know as a normal skin colour or skin pattern on a penis? (been trying to phrase this last sentence better for the last ten minutes, sorry, no can do.)
