I had the same problem but I despite my fears I decided to get examined and then to go along with surgery (the operation was performed two days ago).

I had two "complete" long-term relations so far and I strongly suggest that your boyfriend should seriously consider surgery as the best option. I had absolutely no problems with masturbation since I had full control of the movement and I had located the exact place to apply pressure (near the base of the penis). This way blood-circulation was optimal and I had no difficulty to achieve full erection in short times. Pain was never an issue since the penis head was almost fully covered by the foreskin and there was no direct contact with it. Things are quite different when having sex.

You mentioned the term "one-sided appetite". I quess it refers to the lack of need to have sex becauce of not being able to enjoy it fully. Oral sex was not something great for me, since the penis head was completely covered so the local stimulation was indeed minimal. The presence of smigma within the enclosed space between the foreskin and the penis head may also be a problem for the female partner since it usually gives bad taste due to its stagnation in this area.

The main problem during sex was certainly the inability to maintain the erection after some minutes. As I wrote before, during masturbation blood masturbation is optimal by applying pressure to the lowest part (base) of the penis during the up-and-down movements. During sex this particular point remains outside the vagina, so the effect of phimosis on blood circulation becomes evident. I don't think that the use of a condom will improve the situation. In my case the effect was negative.

The hypersensitivity can be explained by the fact that the penis head remains always covered by the foreskin but I wouldn't say it is something painful. I would describe it just as a sense of discomfort.

The surgical operation (performed using laser) lasted about 20 mins -including the administration of a local anesthetic- and I left the clinic after 30 minutes. It's not painful at all. The hardest part will be to convice your boyfriend to take the first step and visit a doctor.
