My take on the issue (non scientific) is that if you wear underwear that does not promote rubbing of head against the fabric, your glans will retain much of its sensitivity and years later, you will still feel your glans rub if you wear loose underwear.

Some studies have found that actual keratinisation isn't so different between cut and uncut adult males.

Consider your feet. They can still be very ticklish even though they get a lot of pounding every day.
