Tight foreskin

I went to a few urologists about my phimosis and all of them told me to get circumcised. I am 18 years old and no one ever told me about how to pee and clean the foreskin. I can see the glans the opening is like about the size of a dime when I am not erect. None of the doctors are willing to give me topical steroids even though i asked for betamethasone .05%. Is their a way to get this over the counter? or over the internet because if three doctors are not willing to write the prescription, actually none even knew about this cream before I mentioned it to them. I also have begun masturbating properly and it seems each time the opening is getting bigger and bigger. but after words it returns to normal. I really don’t want to get circumcised but I want my glans to come out. Please help me!
