You haven't said that you can't retract, so it does not seem that it is phimosis. Rather it sounds like yeast infection. At least, that's themost likely thing, and it is very simple and easy to treat and then prevent. The hint really is your statement that you keep yourself clean. Doing so overzealously can lead to yeast infections.

You see, yeast is ubiquitous around our bodies, especially the moist parts. Warm-blooded creatures have developed with unseen populations of friendly bacteria that control the potentially damaging stuff like yeast. When we clean with soap we kill a lot of the bacteria and in most areas, like the genitals, that allows yeast to bloom in great profusion.

The treatment - lotrimine or any of the other over-the-counter treatments that women use wherever you are for yeast infections. Use the topical kind, not the suppository (I hope that would be obvious but one never knows). It's the same infection that women use that stuff for and in equally sensitive places.

The prevention - wash only with clean, clear water. A thorough rinse is ample to remove anything that ought to be removed and leaves the good stuff behind (freindly bacteria, natural emollients, etc.)

One other significsnt possibility which comes to mind is exposure to irritating chemicals. If you hsd started a new detergent on your underwear (the rinse cycle usually leaves traces enough to irritate sensitive places) or some sort of a scent or deodorant applied around the willy, then maybe that's the culprit. You can have even developed a sensitivity to a product you had been using all along. If the yeast procedure doesn't work, start looking for the sorts of trace products that your penis may be getting exposed to.
