Answer: This is pretty baffling to me. You have been retracting for years but only long enough to clean it, OR you have remained retracted for years? After only a matter of weeks or months of continuous exposure, the head should no longer be so hypersensitive.
If in fact you have properly identified the foreign matter as smegma, I cannot imagine why it won't come off, if you use enough abrasive force on it. So as not to risk overlooking the obvious, let me explain here that merely allowing water to run across smegma is usually not enough to dislodge it. You need to take a rag or sponge to it.
