Yeah, you definitely need to get them out. Use a needle or pair of tweezers to gently lift the suture material and expose a space in the loop. Then cut it with a pointed scissors. Then you can esaily pull it through the skin. A month is definitely too long for them to be in and their continued presence is delaying the healing.
As for the swelling, I'd say a month is pretty common for there to be some. I was swollen for 5 months believe it or not. Swelling is not a factor to delay the removal of sutures so don't worry about that. When I was worried about my prolonged period of swelling I saw my dctor and he calmly said, "It will eventually go down." Be patient, it will get better.
