Oh. I didn't have any issue with the wad punch vs. drilling, only a method that made it easier to control the squirming rubber whilst the operation was in progress. Senior moment.

BTW, I've done Google image searches for those jpgs on dilators but haven't been able to find them. I believe I ran across them using Yahoo search and switching to the Japanese search engine at the bottom of their home page then searching with the English word "phimosis." It's taking me a while to go through all the links though as this seems to be a subject they are keen on over there.
