Phismosis

I've been told by my son's phisitian that he has Phismosis. He's 4 years old and I was wondering, if any of you suffers the same problem, what should I do?
Should I correct the problem surgically now or should I wait for him to by a teenager? Is it circumcition the only way? How painfull is it when you're having sex?
