I have diabetes and also am not circumcised. The only thing you need to do is keep your blood sugar levels under control. That's all.
I don't pull my foreskin back to have a pee and the only infection I've ever had was when I listened to some bad advice and began to use soap under my oresakin. I got rid of the infection with NIZORAL tables (200 mg once a day for 5 days) available with a prescription from the chemist's and have never had a reoccurance.
One thing I have noticed, is that if you are taking a course of antibiotics for something -- like a sinus infection, for some reason you begin to make extra smegma and you have to be careful to clean your penis. However I think that applies to everyone, not just diabetics. Maybe Paul B. could comment, he's a doctor.
Let me know if you have any other questions. Lots of advice here, much of it from people who were cut when young and almost none from diabetics.
