This situation is directly comparable to the not uncommon procedure of placing a urinary catheter in the bladder through the urethra. If you think your glans is sensitive, you don't know about the inside of your urethra!

Back to that obsession on hygiene! Same as the inside of your urethra - if dirt can't get in there because the opening is tight, then you have nothing to clean out of it. What does work its way in, will be washed out when you pee. Same as the inside of your urethra.

KY or similar is exactly what is used to place a catheter and there is no attempt to replace it during use (which would in any case, merely serve to introduce bacteria). Once the object is in place, there is little shear force on the sensitive areas and enough of the lubricant remains to function. It is not absorbed, and the area being covered, it remains moist. The critical time when the lubricant is needed is during the placement.

It will undoubtedly be uncomfortable to start with, and you may need to wear the dilator for only short times under direct supervision to start with. I venture to say however that you will adapt to it surprisingly quickly.

Consider this in relation to the poor sod who has circumcised - his glans is exposed completely, and irreversibly. He has to adapt to the pressure/ friction on it within a day or two!
