Use them to retract the skin more often. The glans is supersensitive because that is what gives such pleasurable results during normal sexual relations with proper natural lubrication.
Your glans will adapt to the type of touch it is receiving. When you are urinating, skinning back will condition it to stop reacting as it does when you are erect, so start skinning back each time you pee.
You made no mention of methods of hygiene, but could we assume that you pull the skin back to wash? This also allows exposure for conditioning the glans.
Now to the question of inability to pull back when erect. It isn't going to happen until you make it happen. Start by attempting it. Your problem is that you have started a bit later than most guys. You have about 8-10 years to make up, so it may take a bit of time, but with practice and regular exercise, you can get the skin to stretch by pulling back as far as possible each time you have an erection.
There are other methods also, but first try these simple exercise which boys have used since the beginning of time. Of course getting the skin cut off is a solution, but it comes with a price. For sure the hypersensitivity will leave eventually, after the glans is left to deal with the issue for weeks without protection, but the inevitable outcome will be exactly the opposite. Do you want to be left with the possibility of not enough sensitivity, or do you wish to take matters into your control and fix them? You have indicated a desire not to visit the doctor. Does that mean you want to save your foreskin? Perhaps you could explain that.
