I'm not sure where in the world you are, Shane. In the UK, you would first of all need to see your general practitioner. Most surgeries will allow you to nominate which one of the partners you would like to consult, but unless you know the area of specialisation (and the reputation) of each one of the partners you won't be able to make a choice.

My advice is going to cause you some embarrassment (but you're going to have to overcome that anyway, sooner or later. Wait till you're my age and you have to have your prostate gland checked internally -- you'll soon learn to be stoic about these things). If you don't know which doctor in your practice to choose, given the opportunity I would choose a female partner. She will be less likely to have fixed opinions on circumcision, has (obviously) no personal experience of the condition you present, and will probably be more inclined to treat you holistically. She might even be interested in what you're telling her!

As far as the front desk staff are concerned, there's no need to tell them why you want to see the doctor. It's none of their business unless you ask them for a recommendation. But if necessary, just say the word 'phimosis'; it probably won't mean an awful lot to them, and it won't mean a thing to those in line behind you at the desk.
