You've received advice from a competent and skilled general practioner (one who is also knowledgable about surgical techniques) who is familiar with your situation. I have also given you advice on the use of a device to hold open the skin for hours at a time. This advice was given with my personal experience in the use of such devices since 1985. I have also been in personal contact with others who have used devices with success over those several years. Both Paul's ideas and my ideas are based upon sound and sensible real-life experiences and results. You can get busy and make it happen for you, or you can try rationalizing having some essential parts cut off, but continuing to haggle over it will not do the job. If you want the best results, Paul and I have done our best to get the message across to you.
