another tight forskin question

Alright guys, ive been doing a lot of reading but i still have a question.
My forskin is really really tight. There is only a hole big enough to pee thru when soft and erect.

Im going to go talk to my doctor, and try the betamethasone .05% cream.

Has anyone had success with this cream when the forskin was really tight, or is this cream generally more successful when there is atleast some sort of looseness in the forskin?

I really really dont want to have surgury, but im worried the cream wont work because im so tight right now.
