Unfortunately, the best way to know what to do for sure, would be to have a doctor have a look (plenty of people here pretend that they are doctors, but who can say?). An examination is quick and painless. The fact that you have pictures is a good thing too. Your doctor or a specialist will be the best person to get advise from - they've seen this type of thing before and will want what is best for you (regardless if surgical or ointment based treatment).
